The fields are, in order:

O Private customer O Company- or library customer
O Woman    O Man

First name:
Last Name
C/O Address:

    Check this box if you have an address outside Norway

Street name:
Building number:        Building entrance:
Second line of address:

Post number outside Norway:
Place outside Norway (i.e. city):
Choose country:

* = required

Telepone:
Mobile telephone: (* this field is required when using sms for confirmation,
                                    but only norwegian numbers are accepted)

EDIT: My wife tells me the fields for building number and building entrance are picky on what input they accept. so if your address does not fit the wistful thinking of the web designer on how buildings should be numbered and entrances should be named, you might need to stuff that information into the street name field, since that field is not so picky.
