I'm getting bilateral surgery next week. I'm in a similar camp as you. But my FSH is higher and LH lower.  My surgeon says it's around 50/50 for those who have High FSH levels. It depends if the varicocele is the primary cause of testicular failure and unfortunately there is no way to really know if you have already ruled out other factors like Microdeletions, antibodies, and karyotype factors.

Will do.

Just to clarify, I meant basically getting some sort of treatment/repair is the only real way to rule out if varicocele is the primary cause of testicular failure if the other stuff I mentioned has been ruled out.

If your varicocele is unilateral on the left, I would look into embolization. I opted to microsurgery for because it is bilateral. Plus I'm concerned for long term testicular health along with the OAT diagnosis.

He states he has no way of knowing. He has been surprised before. He also publishes research on this and he said that the best responders on the fertility side are those with Normal.FSH levels and says to be realistic.

I am currently on the lower end of the IUI cutoff side of things (5-6 million total motile sperm). He states the goal for me is to get to 10 million motile sperm so I can secure the odds of IUI treatment or the lucky chance of natural conception in the future.
