I really don't understand why this confuses folks.

For starters, Sheldon made that comment in episode 5:3 (in the fall of 2011), and we didn't meet Susan until episode 10:1 - FIVE YEARS LATER. Susan could have lost literally hundreds of pounds over that five year span.

Second, we don't know how old the pictures to which Sheldon referred were. Whose to say they weren't taken 20 years prior to Sheldon's comment, which would mean Susan had TWO AND A HALF DECADES to go from whatever Sheldon saw to the woman we saw.

Comments about the writers forgetting or it being a fluke that they cast Katey Sagal are just silly.

There are a few continuity issues with this show. That was definitely one of them. Like Sheldon learning how to drive and when he got his license comes up multiple times with different stories. Same with Pennys sister - mentioned once and never again. Stuff like that…
