This form governs our relationship from the time I schedule my appointment until the date of my appointment. This form covers one 20-minute meeting. I understand that if I need additional services, WEBA will let me know if WEBA can provide further assistance or other advice such as a different, legal opinion, and if so, under what terms WEBA can offer me those services. If I decide to hire WEBA for additional services, I understand that WEBA is not able to guarantee that I will be represented by the same attorney who I meet with during my consultation. If WEBA discovers that a conflict of interest exists prior to the consultation, WEBA will inform me of the conflict and WEBA will cancel the consultation. WEBA does not print or create hard copies of the documents I provide. WEBA will delete the electronic copies of the records I provide one to two years after my case is closed.
